NATURE OF SERVICES Life Moments Child Care provides short-term on-site childcare services for private events. Services are temporary, occasional, and incidental to the event and are not licensed childcare under North Carolina law. Parents/guardians must remain on-site at the event or immediately reachable at all times.

SERVICES PROVIDED

Supervision of children during event hours
Age-appropriate activities and entertainment
Basc care (excluding medical or specialized care unless agreed in writing)

STAFFING & SUPERVISION

Caregivers will be assigned based on the number and ages of children attending the event. While reasonable supervision will be provided at all times, Life Moments Child Care does not guarantee constat one-on-one supervision unless specifically contracted.

HEALTH & SICK POLICY

Care may be refused for any child exhibiting:

Fever within 24 hours of event
Vomiting/diarrhea within 24 hours of event
Contagious illness or unexplained rash

If a child becomes ill, the parent will be contacted for immediate pick-up.

LATE PICK-UP POLICY

Parents/guardians are expected to pick up children promptly at the agreed upon end time. A late fee of $25 per 15 minutes will be charged for late pickups to compensate staff.

CLIENT RESPONSIBILITIES

The client agrees to provide a safe, clean, hazard-free childcare area.

The client agrees to inform guests of childcare availability.

The client agrees to ensure each child is registered by a parent/guardian.

The client agrees to ensure parent(s)/legal guardian(s) remain on-site or immediately reachable.

ASSUMPTION OF RISK

The client acknowledges that group childcare activities involve inherent risks, including but not limited to trips, falls, and interactions between children. The client voluntarily assumes all such risks on behalf of participating children.

LIABILITY WAIVER & INDEMNIFICATION

To the fullest extent permitted under the laws of North Carolina, the Client agrees to release, waive, defend, and hold harmless Life Moments Child Care, its owners, employees, and contractors from any and all claims, damages, or liabilities arising out of participation in childcare services, except in cases of gross negligence or willful misconduct.

INSURANCE

Life Moments Child Care maintains general liability insurance. Proof is available upon request.

CANCELLATION POLICY

Deposit is non-refundable. Cancellation within [14] days may result in additional charges.

GOVERNING LAW

This Agreement shall be governed by the laws of the State of North Carolina.

ENTIRE AGREEMENT

This Agreement constitutes the entire understanding between the parties.

By signing this agreement, I understand that childcare activities include physical play, interaction with other children, and use of toys and equipment, which carry inherent risks including but not limited to minor injuries from play.
